Phantom Wurm, Creature — wurm spirit, designed by Alan Pollack first released in May, 2020 in the set Time Spiral and was printed exactly in 2 different ways. It's currently being selled by the minimum price of NZ$ 0.61.
Phantom Wurm could be a valuable addition to a deck focused on +1/+1 counters, such as a green-white or green-blue proliferate deck in Magic: The Gathering. Its ability to prevent damage and effectively act as a damage sponge can be useful in controlling the board and protecting other important creatures. However, there are more efficient options like Vorapede or Verdurous Gearhulk that offer better value for their mana cost and abilities, making Phantom Wurm less likely to see play in competitive decks.

If an effect raises Phantom Wurm’s toughness so that it remains on the battlefield without any +1/+1 counters on it, Phantom Wurm’s ability prevents all damage that would be dealt to it even though you can’t remove a +1/+1 counter from it.

If Phantom Wurm is dealt damage by multiple sources at once (most likely because it was blocked by multiple creatures in combat), all of that damage is prevented and you remove only one counter from it.
